/Salary: $44,458 to $48,713 Annually Salary is based on education & experience

Job Description: LVN implements professional & administrative nursing services under the direction of the RN in the IDD Services programs of ICF-ID, HCS, and TxHmL; is responsible for maintaining a competent & professional level of nursing care to persons with intellectual and developmental disabilities who receive services and supports through programs & for monitoring the delivery of medical related training, which include, but not limited to lab screenings, physical exams, medication education/training including side effects, medication administration, liaison with pharmacies to ensure timely and accurate medication orders and to ensure safe and effective care is delivered; conducts focused health/nursing assessments within LVN scope of practice; coordinate follow up with provider staff, the individual, and/or family regarding client medical issues/needs to ensure effective and efficient medical treatment/services is received as ordered/directed by healthcare personnel; provides oversight and ensures proper implementation of the medical/nursing outcomes for the individual’s receiving ICF-ID, HCS, and TxHmL services; requires working closely with the interdisciplinary/service planning teams to facilitate and implement nursing treatment plan and nursing treatment recommendations. This position must comply with CMS’s Omnibus COVID-19 Health Care Staff Vaccination rules published in the Federal Register on November 5, 2021. COVID-19 vaccine immunization is required and must be completed prior to hiring.
Education: Graduate of a professional nursing education program.
Experience: Prefer experience working with persons with intellectual and developmental disabilities in a community-based setting and knowledge of ICF-ID, TxHmL and HCS Medicaid Waiver Programs rules, billing guidelines, standards, and methods. Requires skill in determining individuals needs assessment with the ability to respond to changes and ability to work independently as well as with a team; good organizational skills are necessary.
Hours of Work: Generally, Monday—Friday 8am-5pm; adjustments in work hours warranted to meet the needs of the program. Will require travel within MHMRABV’s seven county catchment area.
Licenses/Certifications: Licensed in the State of Texas to practice as a Licensed Vocational Nurse, as governed by the Texas Board of Nursing. Must have reliable transportation, a safe driving record, a valid driver’s license and must be insurable by Agency insurance.
Physical Requirements: Ability to drive a vehicle, communicate effectively both verbally and written, ability to sit or stand for extended periods of time, simple grasping, walking, operating office equipment include computer skills, ability to see and hear (with aid), ability to read, write, count, maintain confidentiality, and the ability to work closely with others, ability to lift/carry under 30lbs; ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position.
Facility: IDD Waiver/ICF-IDD Services, Bryan TX; Requires travel within seven counties service area.
