Salary: $40,275 to $49,081 Annually                           Salary is based on education & experience

Job Description: As a LIDDA Enhanced Community Coordination Service Coordinator (ECC), provides crisis prevention and management, monitoring, assessment, and service planning and coordination through the ECC Demonstration Project to individuals with intellectual and developmental disabilities (IDD) residing in an institution (i.e., state supported living center (SSLC) or nursing facility (NF)) who are transitioning to a community Medicaid waiver program or community intermediate Care Facilities for individuals with an Intellectual Disability or Related Conditions (ICF/IDD) residing in Brazos, Burleson, Grimes, Leon, Madison, Robertson, and Washington Counties.  ECC Service Coordination involves accessing and monitoring medical, social, educational, and other appropriate services that will help an individual achieve a quality of life and community participation acceptable to the individual which is based on a person-centered individual service plan and  includes proving information of available community living options, services and supports, enhanced pre- and post- transition services; ensuring opportunities to visit community resources; provides intensive support to ensure an individual’s successful transition to the community to prevent admission or re-admission to an institution; provides detailed information to an individual and LAR about community options and alternatives; provides increased coordination and interaction with an institution’s interdisciplinary team and the assigned relocation specialist, as appropriate to the individual’s needs and situation; conducts service planning team meetings at least quarterly, or more frequently if there is a change in needs or requested; conducts at least monthly face-to –face visits with an individual or monitors the delivery of all services and supports while the individuals is in an institution and at least one year after the individual is in an institution and at least one year after the individual has transitioned into the community, adhering to HHSC performance measures and contractual requirements.
Education: Requires Bachelor’s or advanced degree with a major in a behavioral, social, or human service field, including but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice.
Experience: Prefer experience working with individuals with IDD. Prefer Qualified Intellectual Disabilities Professional (QIDP) required education and experience requirements; must be able to work independently as well as with a team. Good organizational skills are necessary.
Hours of Work: 8:00am—5:00pm Monday through Friday; may require after-hours flexibility to meet the needs of the person in service/program. Travel within seven county service  area
Licensure/Certifications: Must have reliable transportation and a safe driving record. Must have a valid Texas driver’s license and be insurable by Agency insurance.
Physical Requirements: Ability to drive a vehicle, ability to communicate effectively; knowledge of case management principles, objectives, standards and methods; knowledge of Medicaid waiver program services and non-Medicaid waiver program services;; skill in determining individual needs assessment and the ability to respond to changes required; ability to comprehend and implement program goals; ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position.
