Salary: $35,808 to $43,637 Annually

Job Description: Serves as a Local Intellectual and Developmental Disability Authority’s (LIDDA) representative of the Community Living Options Information Process (CLOIP) Program to provide standardized information, explanation, and educational materials of available community living options to all individuals ages 22 and above who are residents of Brenham State Supported Living Center and the residents’ Legally Authorized Representative. For persons interested in/desiring community placement in a specific location, coordinates and facilitates details including arranging sample community tours prior to annual servicer planning team (SPT) meetings. Submits results of tours, explanation of community living options and interviews with individuals, LARs or advocates to individual’s SSLC SPT. Attends annual SPT meetings of all assigned individuals to discuss community living options. Also provides all information gathered to the individual’s designated LIDDA Documents above activities as required by Health and Human Services Commission (HHSC) and MHMRABV; Must have access to a personal vehicle, will require routine travel to Brenham State Supported Living Center. For eligible nursing facility residents, provides Habilitation Coordination for Preadmission Screening and Residential Review (PASRR), within all seven counties, including: Brazos, Washington, Burleson, Grimes, Leon, Madison, and Robertson Counties. Service Coordination involves accessing medical, social, educational, and other appropriate services that will help an individual achieve a quality of life and community participation acceptable to the individual which is based on a Service Plan (SP). The Service Coordinator will negotiate and facilitate the array of services and supports needed to address the individual’s personal goals and desired outcomes as identified through the Service Coordination Assessment and Service Planning Team. The Service Coordinator is responsible for ensuring planned services and supports are implemented and appropriately documented. The Service Coordinator will advocate and ensure consistent interactions with nursing facility and other agencies as necessary.
Education: Bachelor’s Degree from an accredited college or university with a major in social, behavioral, or human services field.
Experience: Requires one year experience working directly with persons who have and intellectual or developmental disabilities/meet QIDP credentialing requirements. Prefer experience with prior casework or service coordination experience working with individuals with intellectual and developmental disabilities: must be computer literate, requires excellent verbal and written communication skills, requires excellent organization skills, including the ability to meet rigid timelines and manage multiple priorities.
Hours of Work: Monday—Friday, 8:00am—5:00pm; some flexibility maybe required occasionally.
License/Certifications: Valid Texas driver’s license & must be insurable by Agency’s insurance.
Physical Requirements: Ability to drive a vehicle, communicate effectively both verbally and written, ability to sit or stand for extended periods of time; good computer skills a must; ability to lift/carry under 30lbs; ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position.
