Program Specialist II Transition Support Specialist

Salary: $45,148-$49,467 Annually

The transition support specialist will provide intensive transition support and care coordination for youth and adults, facilitating their transition from an inpatient facility to a community setting, with a focus on state funded beds including contract beds and state hospitals. The goal is for individuals to transition successfully to their community of choice, while promoting the recovery process. This position will be responsible for fostering working relationships with external providers and facilities as well as conduct community presentations and educational opportunities to inform about transition support services.
Education: Requires Bachelor’s Degree in a behavioral, social sciences, humanities or education field.
Experience: Prefer experience working with individuals with behavioral health issues.
Hours of Work: Typically 8:00am-5:00pm, Monday—Friday; need to have a flexible schedule to accommodate needs of program and families. Crisis On Call required as agency need dictates.
Licenses/Certifications: Ability to be credentialed as Qualified Mental Health Professional. Valid Texas driver’s license and are insurable by agency insurance. Continued employment is contingent upon successful completion of Adult Needs and Strengths Assessment (ANSA) certification.
Physical Requirements: Light lifting under 15lbs., moderate carrying 15-44lbs., simple grasping, walking, sitting, standing, operating a motor vehicle, operating office equipment including a computer, ability to see and hear (with aid), ability to read, write, and count, ability to maintain confidentiality, ability to work closely with others, and the ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position
