/Salary: $37,598 to $45,819 Annually Salary is based on education & experience

Job Description: This position provides Pre-admission Assessment Screening and Resident Review (PASRR) services, which includes screening and evaluation to determine whether an individual identified as having a mental illness needs nursing facility services; identify alternate placement options when applicable; and identify specialized services that may benefit the individual. Duties also include completing intakes with positive PASRR individuals and participating in interdisciplinary meetings with nursing facility staff to coordinate appropriate care and assess needs; assisting families and other community providers with referrals to the PASRR program ; providing specialized services to eligible individuals based on ANSA and client needs and preferences; monitoring TMHP LTC portal daily to ensure all requested services are provided within required timelines; this will be primary clinician for all PASRR clients in all 7 counties.
Education: Requires Bachelor’s Degree in a Behavioral, Social Science, Humanities or Education field.
Experience: Requires experience working with individuals with mental illness, must be able to work independently as well as with a team. Good organizational skills are necessary.
Hours of Work: 8:00am—5:00pm, M-F; adjustments in work hours may be warranted to meet the needs of the program. Crisis On Call required as agency need dictates.
Licensure/Certification: Ability to be credentialed as a Qualified Mental Health Professional. Valid Texas driver’s license and are insurable by agency insurance. Continuing employment is contingent upon successful completion of Adult Needs and Strengths Assessment (ANSA) certification as well as Illness, Management and Recovery (IMR) certification.
Physical Requirements: Ability to drive a vehicle; communicate effectively both verbally & written; ability to sit or stand for extended periods of time. Good computer skills are necessary.
